FDA Announces ReliOn Insulin Syringe Recall

On November 5, 2008, the U.S. Food and Drug Administration reported an nationwide recall of incorrectly labeled lot of ReliOn Insulin syringes. The recall was announced by Tyco Healthcare Group LP due to a possible mislabeling of the dosage in each syringe, resulting in a possible overdose delivery of insulin.

Potential Fatal Overdose



The ReliOn Insulin syringe is a sterile, single-use, disposable, hypodermic syringe with a permanently affixed hypodermic needle. It consists of a single barrel, plunger rod, and hypodermic needle affixed to the tip of the syringe. The recall is being issued due to a potential mislabeling of the quantity of insulin in each syringe; syringes containing as much as 100 units of U-100 insulin were erroneously marked with a label for a U-40 syringe. The use of these mislabeled syringes could result in an overdose situation with the patient receiving as much as 2.5 times the intended dose. This could lead to hypoglycemia, serious health consequences, and even death.

The recalled syringes are distributed by Can-Am Care Corp and were sold only by Wal-Mart Corporation at Wal-Mart stores and Sam's Club stores under the ReliOn brand name. The manufacturer distributed 4,710 boxes of the now-recalled syringe, amounting to 471,000 individual syringes. They were sold at Wal-Mart stores and Sam's Club stores between August 1, 2008 and October 8, 2008. Wal-Mart issued a statement asking all users of the ReliOn 31-gague, 1cc syringes to return all syringes labeled as 100 units to their local Wal-Mart store or Sam's Club pharmacy for a replacement. Additionally, Tyco Healthcare issued a statement on their ReliOn website announcing that customers who had purchased the incorrectly labeled product would receive a complimentary box of replacement ReliOn syringes.
